Kentucky only offers one type of sales tax permit called a Sales and Use Tax Account. There currently is no charge to apply for a Kentucky sales tax permit.

To get started, you can either print and fill out the paper application or you can fill it out online. Either way, you can fax the completed application to the state of Kentucky at (502) 227–0772.
